These terms and conditions form a legal agreement between DTC Medical Weight Loss and you, the individual concerning your access to and use of the various services offered through the websites and platforms as described below. Use of the website and any associated platform constitutes your acceptance of the terms and conditions set forth by DTC Medical Weight Loss.
DTC Medical Weight Loss and its affiliates own and operate websites including DTCMedicalWeightLoss.com, as well as other related websites that may link back to these sites and others. This is collectively known as “the websites”. The websites are facilitators between registered users and providers in order to provide services to users which could include general information pertaining to erectile dysfunction and available treatments which could include the prescription and use of either Viagra® (Sildenafil) or Cialis® (Tadalafil) tablets; administrative support pertaining to medical records and/or telecommunications or support services for the purpose of communication, consultations, assessments and treatments of patients by our providers.

DTC Medical Weight Loss and its affiliates are located in the state of Colorado. By using our website and the services therein, you’re affirming to the fact that, at the time of your visit with the provider, you’re located within the state of Colorado.
DTC Medical Weight Loss is NOT a Medical Practice
DTC Medical Weight Loss is not a healthcare provider and the providers in our service are not employed in that capacity. While we facilitate communication between users and providers, we do not provide medical advice or care, whether immediate or ongoing. All medical care is provided through Weight Loss MD or other providers we have relationships with although they are separate from DTC Medical Weight Loss. The provider seen may deny treatment if they determine you’d be better served by a local provider or for any other reason. The advice and prescription that may be provided to you is based on the information that has been provided by you and their professional judgement. DTC Medical Weight Loss does not provide medical advice and we do not influence nor control the advice or treatment provided by your healthcare provider.
DTC Medical Weight Loss is NOT a Pharmacy
All medication is prescribed by licensed medical providers and shipped to you direct. None of these services replace your relationship with your physician. DTC Medical Weight Loss and its providers do not diagnose, treat, or screen for diseases such as cancer or other diseases related to the prostate, urogenital area or other part(s) of the body. DTC Medical Weight Loss and its providers only provide advice and prescriptions relating to erectile dysfunction. Any medication prescribed for this is not intended for the treatment and/or diagnosis of the underlying causes of erectile dysfunction, only the treatment of symptoms.

You must be 35 or older to be eligible for our Services
If you are under 35, you are not eligible to receive services through our website, receive a prescription or meet with our providers. Instead, you should see your PCP for services rather than use our services at this time. By accessing our site and becoming a registered user, you agree that you are over the age of 35. Telemedicine; Benefits, Risks; Your Consent
Telemedicine is an electronic method through which registered users are connected with providers who evaluate patients and provide prescriptions from other locations. Communication may include transmission of medical records and other personal health information (PHI). This may include dynamic intake forms, live 2-way video or telephone calls, among other methods. DTC Medical Weight Loss technology platform is designed to contain security protocols to protect your PHI. Through using our platform, you are consenting the information provided is true and accurate to the best of your knowledge, it is for you and only you and is not falsified in any way. You also consent to notify DTC Medical Weight Loss of any inaccuracies, errors, omissions, or inconsistencies.
Privacy
Your privacy is of the utmost importance to DTC Medical Weight Loss and its providers. Please see our privacy and security policy which explains how information is used and collected. If you use our website or services therein, this constitutes acceptance of the privacy and security policies which can be found on our website. These policies may be amended at any time without notice. Fees and payment
You are solely responsible for the products and services purchased through DTC Medical Weight Loss’s services. DTC Medical Weight Loss bills based on the option purchased and coordinates shipments of your refills based on this. The payments quoted are based in U.S. dollars. Payments may be processed through a third-party payment processing service and you’re authorizing DTC Medical Weight Loss to charge you through this service. You have warranted that you are authorized to present and use the payment methods provided to DTC Medical Weight Loss. If your payment method is declined or rejected by the bank, this will constitute automatic cancellation of your services through DTC Medical Weight Loss. DTC Medical Weight Loss may terminate your account and right to access the website and services therein at any time, for any reason with or without notice to you. We will not be liable to you or any third party should we choose to exercise this right.

Release of Liability

Indemnification

Assignment
DTC Medical Weight Loss may choose to transfer, sell or assign to third parties its rights under these terms and conditions and any rights related to your relationship with DTC Medical Weight Loss including, without limitation, transferring your account to include any PHI or other information provided by you or on your behalf to DTC Medical Weight Loss. Providing that these terms and conditions are binding upon and acclimate to the benefit of the DTC Medical Weight Loss’s successors or assigns. You may not assign your rights or your relationship with DTC Medical Weight Loss, and any attempt to do so will be null and void.
Modifications to these Terms and Conditions
DTC Medical Weight Lossreserves the right to make changes, add or remove provisions to these terms and conditions, privacy or security policies at any time by posting new terms and conditions, privacy and security policies on the websites. It is recommended that you read these policies each time you access the website and services therein. If you object to any changes, you must cease using the websites and services therein immediately.
